How to use Chlorthalidone with Nephrosis
Kidney diseases have become more frequent over the years. These diseases if not managed well all have a negative impact on the kidneys thus the need for drugs administration of drugs that try and prevent kidney failure. One of the drugs is called Chlorthalidone. It is a diuretic that is usually prescribed for various conditions namely, high blood pressure and conditions that have the effect of causing fluid retention in the body. The drug has been proven over the years to be more effective than the commonly used thiazide which is hydrochlorothiazide.
Kidney diseases are complicated conditions. If misdiagnosed or poorly treated they result in permanent damage to the kidneys. Nephrosis can also be referred as Nephrotic Syndrome which is basically a group of diseases that attack the kidneys and thus cause the kidney to lose its ability to excrete excess fluids in the body and filter waste products from the body. The system is important in controlling the electrolyte balances in the body thus there is usually protein imbalance among other electrolytes.
The treatment of Nephrosis is dependent with the cause of the condition. In a situation where the cause is known, such as diabetes and hypertension, treatment is usually targeted at controlling the causative disease. There can be situations however where the cause of the Nephrosis is unknown. The treatment here is targeted at controlling the symptoms that are present. As described above Nephrosis is not just a single condition but a group of conditions that generally affect the kidneys. Though these conditions are a significant number, they mostly have the common effect of causing fluid retention in the body which eventually leads to oedema.
The mode of action of Chlorthalidone is similar to that of thiazide diuretics in the fact that it causes diuresis to increase, with increased excretion of Sodium and Chloride. When there is trauma to the kidney by diseases, there is alteration in how salt and water are excreted by the organ. Failure of Sodium excretion results in increase in extracellular volume as the glomerular filtration rate falls. When there is further decrease in kidney function, the resulting effect is causing peripheral oedema or pulmonary oedema.
The use of the drug is thus important for administration in these cases. Thus Chlorthalidone has been proven to be effective in patient who are have low Glomerular filtration rate. It increases the rate of diuresis, which is important in the treatment of Nephrotic Syndromes. Most of the nephrotic conditions cause high blood pressure due to this fluid retention. Chlorthalidone is used to lower elevated blood pressure.
The risk for cardiovascular disease is high when blood pressures are elevated and this drug can reduce the chances of cardiovascular disease by reducing the extracellular fluid volume. The effects of Chlorthalidone are considered to be long acting than other diuretics of the same class such as hydrochlorothiazide. It produces the desired effect of controlling high blood pressure for longer than other thiazide diuretics.
Chlorthalidone is found in oral form. The tablets are available in 25 mg and 50 mg. For patients with oedema caused by Nephrotic Syndrome the initial dose recommended usually is 25mg per day. They are however patients who use much higher doses of 50mg per day. However, the dosage above this level is discouraged because it does not produce the desired greater effect.
This drug should never been used in a patient with known sulphonamide or thiazide diuretic hypersensitivity. Chlorthalidone should be used with caution for patients that have been diagnosed with severe renal diseases as it causes hypovolemia which can lead to azotemia. The drug is also contraindicated with patient with hepatic diseases as the slight alteration in fluid and electrolyte can cause precipitate hepatic coma.
Nephrotic syndromes are a major cause of Chronic Kidney Failure. These diseases have undesirable effects that affect the normal living of the victim. Chlorthalidone is one of the few drugs that can help in decreasing some of these undesirable effects.
