What is Mesalamine

Mesalamine is an anti-inflammatory drug which has been used in the treatment of Ulcerative colitis and other inflammatory conditions in the body. Mesalamine reduces effects of ulcerative colitis such as bleeding in the rectum, passing of watery stool and pain in the stomach. Other Names include: Asacol HD, 5-ASA, mesalazine, Pentasa, Delzicol, Lialda, Apriso Class Mesalamine is in a class of drugs known as 5-Aminosalicylic Acid Derivatives. Other drugs in this class are olsalazine, Asacol HD, Dipentum, Giazo, Lialda, and sulfasalazine.Mode of Action Mesalamine acts to curb prostaglandin release which causes inflammation of cells. It also blocks leukotriene synthesis and releases in the large intestines. 5- aminosalicylic acid is the active component in Mesalamine.


Once taken, the immediate release form of Mesalamine  is absorbed rapidly while the extended release mesalamine has a 25% bioavailability rate. Improvement noted in the colon after taking the drug is usually after 7 days to approximately three months. Mesalamine reaches peak plasma levels in 4hrs (apriso), pentasa takes 3 hrs, Mezavant takes 8 hrs while Asacol HD takes approximately 13 hrs. 5- ASA binds to blood protein albumin at 43% while N-Acetyl-5-ASA binds at 78%. Unlike other drugs that go through the hepatic system alone, mesalamine is metabolized partially in the colon. Mesalamine acetylated in the large intestine wall and the hepatic system to form a metabolite in the inactive form which is N-acetyl-5-aminosalicylic acid. The half-life of the active form 5-ASA is 1-9 hrs while that of the inactive form N-acetyl-5-aminosalicylic acid is 3-16hrs. The majority of Mesalamine is later eliminated through the biliary duct at 72% and some of it through the urine (20-30%)

How To Use And Dosage

Dosage For Ulcerative Colitis 160 mg TDS orally when taking Asacol HD0.8g TDS orally when taking Delzicol 240 mg – 480 mg orally for up to 8 weeks once daily when taking Lialda 100 mg four times a day orally when taking Pentasa. This should be taken for 8 weeks.To prevent remission or reoccurrence of the Ulcerative colitis, it is recommended that mesalamine tablets be taken as follows 150 mg of Apriso orally daily 160 mg per day orally of Delzicol 160 mg per day orally of Asacol HD 240 mg of Lialda orally daily with food.100 mg of Pentasa orally four times daily for 8 weeks.

How do you take Mesalamine?

As directed by your doctor, take this medication orally usually its taken 3 or 4 times on a daily basis. Mesalamine capsule can be taken with meals or without while the mesalamine tablet is taken strictly with food. When taking Mesalamine, do not try to chew or crush it with your teeth while in the mouth instead, swallow it as a whole. In doing so, Mesalamine is able to arrive at the colon safely where it is acetylated.The dosage of Mesalamine might not be the same for all the patients. Side Effects
